Partnering for Better Health
Health coaching is a preventive service that doctors can recommend for their Medicare beneficiaries with multiple conditions like high blood pressure, high cholesterol, diabetes, heart disease, and arthritis, to help them make better decisions about diet and activity.
The Care Collaborative is working in partnership with rural Kansas hospitals to develop new treatment models that result in better patient outcomes and reduced healthcare costs. Nearly 1 in 4 Kansans die from heart disease or stroke alone every year, with rural communities having the highest mortality rates and among highest healthcare costs in the nation.
This program focuses on coordinating care by developing and implementing shared clinical guidelines, and delivering telehealth services, follow-up care and teamwork across all levels of care.
Using local and regional resources, the program will establish care coordinators and health coaches in rural communities who engage at-risk patients in maintaining and improving their health.
Performance is continuously evaluated to measure progress in achieving the goal of improving patients’ outcomes, while reducing healthcare costs.
